Business Membership Application

Note: For faster processing, please review and follow all instructions. Your Business Membership WILL NOT be opened without ALL required and completed documentation. Incomplete Business Membership Applications will be declined after 2 business days. Applicants whose applications are declined due to incompleteness are always welcome to reapply at their convenience.
Business Membership Eligibility
For business membership eligibility, Owner(s) of the Business must provide us the business Tax ID number, as well as the business license number.
House Material Mart requires one of your business administrative officers would be able to talk with us for verification before opening the account. Physical appointment might be also requested at the convenient location (normally our warehouse or your office) depends on the type of business account that we are setting up.
All House Material Mart business accounts must be in good standing to open a Business Membership. Some business accounts will be requested to sign an NDA agreement in order to access further pricing details. Once we set up your business account, the account must remain in good standing for the duration of the Business relationship with House Material Mart. The failure of any officer to maintain this status will result in the closure of associated Business accounts if the business account is not active for the next 45 days.
How to Establish a Business Membership
Required Business Entity Documentation
Your business Tax ID number and a copy of your business license page.
Tax exempt form will be requested if you have resale license.
Please ensure all signatures are provided and appropriate documentation is included with your Application.
